Insurance and Payment Options

Dr. Hoffman accepts private pay and insurance as options to pay for services.

Insurance

Dr. Hoffman is on the following insurance panels:

• Aetna
• Anthem
• Blue Cross/Blue Shield
• Cigna
• Medicare
• TriCare
• United Behavioral Health

Out of Network

Dr. Hoffman does assist with out of network insurance claims as well. With out-of-network payments, payment is due at the time of service. The person who does billing for Dr. Hoffman will submit the claim; however, Dr. Hoffman cannot guarantee that the insurance company will accept the claim. If the insurance company does accept the out-of-network claim, you will be reimbursed from the insurance company. It is the client’s responsibility to check to make sure that their insurance company will accept the out of network claim. If the insurance company does not accept the claim, clients are remain responsible for the payment at the time of service.

Payment Options

Dr. Hoffman accepts cash, check, and credit card payments. Credit card payment is available for private pay clients and for co-payments. Credit Card payments can be made at the appointment or through an online secure client portal.
